We get to know our residents and their families to better understand each resident’s individual needs and wants, allowing us to formulate a plan of care that best suits them. As a team, we acknowledge the reality that our residents live in, redirect when necessary, modify the environment, and change our expectations, allowing us to fulfill our commitment to keeping all of our residents happy, comfortable, and functioning at the highest level possible.

We value and facilitate therapeutic relationships, so your loved one develops a sense of security and belonging. Because of our enhanced training and supportive culture, we maintain a 7% antipsychotic and antianxiety medication rate, whereas the national average is 20%. This in itself speaks volumes about our personalized care and services as memory care facilities often have a much higher medication rate than the national averages. We have also proudly maintained a resident retention rate of 99.9% for over 20 years. Simply stated, our residents are happy as well as their family members.
Impeccable State Surveys
It is our mission to provide quality care to the people we care for, which is reflected by being accredited as deficiency free for over 12 years and 15 state surveys in consonance with the State of Wisconsin licensing board.
How does this compare to our competitors? The national average of state citations per visit is over six violations is over six violations per visit, which means that Heartwood has had zero citations in the time the average facility would have had over 100.
